Multithreading Mutex indicates loss of synchronization -- you should restart the server at earliest possible point (after users have ended sessions).
This error has not been seen following significant Inventu and customer testing as well as large configuration production execution--but it nevertheless is issued if an error should occur--please report with a Support Ticket should it occur on your server.
